St. Joseph Hoag Health's Center for Health Promotion's Diabetes Program Merits High Standards for Care

St. Joseph Hoag Health’s Center for Health Promotion’s diabetes program located in both Brea and Santa Ana has been awarded continued recognition from the American Diabetes Association. This is a high achievement in diabetes care and demonstrates that the program offers high quality education services to the patients it serves.

A program achieving this national standard has demonstrated a greater consistency in the quality and quantity of education offered and is delivered by knowledgeable health professionals, who can provide state-of-the-art information about diabetes management for participants.

Self-management education is an essential component to diabetes treatment. Participants in the program learn self-care skills that will help them better manage their diabetes and reduce risk of complications. Unnecessary hospital admissions for acute or chronic complications can be prevented through this type of self-management education, as well as improved quality of life.

The Center for Health Promotion teaches their classes in fun and interactive sessions. “We do not lecture to patients," states Susan Watkins, RD, CDE, the program manager. “We use guided conversation and discussion on important topics." Maps and cards are also used as a fun way to learn and illustrate information effectively. “When you make it interactive, patients can learn and connect with each other, and we have seen many friendships brew as a result of our program," states Watkins.

A recent patient Gabe, referred by Dr. Jonathan Anderson, attended the program this summer. He lost 20 pounds, and his A1C went from 8.5 to 5.3, fasting glucose from 305 to 126, and his cholesterol levels dropped dramatically during the program. He feels better than ever and is grateful he had this program to help him learn positive life changes to manage his condition. He says that he is forever grateful to his doctor for sending him to this program and to his dietitian for all of her education and support.
